Winning six medals, Duncan was star of the pool at Gold Coast 2018, becoming Scotland's most decorated athlete at a single Games and the first Scottish swimmer to win gold in the 100m Freestyle.

His performance showed the progress he had made since winning silver for Team Scotland in the 4x200m Freestyle Relay at the Glasgow 2014 Commonwealth Games at the age of 17. He followed up with gold at the 2014 Youth Olympics, but it was the following year that he hit the headlines, taking three gold and three silver medals at the inaugural European Games in Baku. 2015 also saw him make his World Championship debut, swimming in the heats for the gold medal winning GB team in the 4x200m Freestyle Relay.

The Rio 2016 Olympic Games saw a phenomenal performance from Duncan as he won silver in both 4x200m Freestyle and 4x100m Medley relays and took 5th in his individual 100m Freestyle. In 2017 he won World Championship gold and silver in the relays and took 4th and 5th places in his individual Freestyle events. He was back on the World Championships podium with gold in the 4 x 100m Medley Relay and a bronze in the 200m Freestyle, his first individual global medal.

Duncan took home three golds and a silver from the 2018 European Championships before earning his first World Championship medal in an individual event in 2019 by taking bronze in the 200m Freestyle. It was his relay performance, however, that really stood out as he produced the second-fastest relay split of all-time, and fastest in a textile suit, on the last leg of the 4x100m medley to bring the British team home to gold.

The European Championships in 2021 saw him claim two gold and three silver medals before he went on to become the first British athlete ever to win four medals at a single Olympic Games with gold and three silver at Tokyo 2020. During 2022 Commonwealth Games in Birmingham, Duncan took five medals home: two golds in Men's 200m Freestyle and Men's 200m Individual Medley, and three bronze for Men's 100m Freestyle, Men's 400m Individual Medley, and Men's 4 x 100m Medley Relay.
